- You can use these on-line self-assessment forms and see the results without having an account however if you would like to submit your assessment to us for further support you will be prompted to create an account and provide further information.
- Once you have an account and we have verified your details you can submit forms to us and be given access to your dashboard.
- You will also be able to save forms and return to them later.
- Your dashboard will show your details, your messages and copies of your submitted assessments.
Help to manage your account
- To be able to register for an account you will need to have a valid email address.
- Once you have entered your registration details you will be sent a verification code to the email address that you have provided.
- You will need to enter the verification code in order to finalise registration.
- Verification codes are only valid for 10 minutes so make sure you add the code before it expires.
- Your verification code will be sent from ‘Redcar Adult Social Care Portal’.
- Always check your junk folder as sometimes the verification codes can show up there. It can also be useful to double-check the email you have provided.
- If you don't receive a verification code within ten minutes you can request another code by selecting the option to request a new code. This will be sent to the email address you have provided.
- If after requesting a new code you still do not receive it please contact us on 01642 771500 during office hours and we will be happy to help. Our offices are open 9am-5pm Monday to Thursday and 9am-4:30pm Friday.
- You can navigate through the forms by completing any mandatory fields and using the ‘next’ button near the bottom of the form.
- If you need to go back to the page you were just on you can use the ‘previous’ button or the menu on the left-hand side of the form.
- Try not to use the browser 'back' button as you will leave the form you are working on and lose the information you have already entered.
- For each question select the answer that best describes your situation.
- You do not need to answer every question, you need only complete what is relevant and mandatory.
- This symbol * means that the question must be answered before you can continue.
- To save a form you will need to create an account or log in using your account details.
- Select the ‘save for later’ button to save and return to later.
- Your form will be saved for 30 days, and you will receive an email confirmation.
- When you log in you can select the ‘saved forms’ button in the top right-hand corner of the screen to find your saved form.
